Only one registration per recipient (regardless of whether you are receiving one award or more than one award) - Other awards to be presented are listed later in the booking.
When registering please fill in the following information:
NAME and ADDRESS should be the name of the young person receiving the certificate (the certificate will be printed with the name entered on the booking)
-Citation - a paragraph stating what you did to get your award (this will be read out as you step onto stage) if you have more than one award please leave a space then put any additional citation below the original citation
-OTHER AWARDS - please indicate here if you have more than one certificate to be presented (ie use the options to show the other certificates such as DofE or ESYL Certificate)
If you are being presented with a DofE certificate you must supply the certificate to the organisers on arrival at the ceremony as we can't source these certificates.
