We know that one of the most challenging times for young people with diabetes is when their care shifts from the child to adult services. Having a positive experience during transition is critical for ensuring that they continue to engage with their clinics and even how they manage their diabetes.

Presented by our Together Type 1 team, we will delve into how meaningful involvement from young people can support the successful design and improvement of transition services. You will hear from two healthcare professionals who share their successes and challenges experience working with young people as well as from our own Diabetes UK Young Leaders. The session will wrap up with a panel discussion with the opportunity to ask questions.

From this session you will:

  • Improve your understanding of the importance of involving young people in transition services,

  • Learn practical ways that you can create meaningful involvement opportunities and how Diabetes UK can support this,

  • Connect with and hear from other healthcare professionals who work in the transition space.
