Intensive 245D providers need to write compliant, comprehensive documents for each person that they support. In this 7-hour class, we will review the details on how to write some of the most complex documents including the Individual Abuse Prevention Plan (IAPP); Self-Management Assessment (SMA); the Intensive Addendum, and Outcomes. We will give you the tools for success to help you feel confident in developing these critical documents. We will also discuss why these documents exist and how they contribute to your person-centered supports.
CEU Contact Hours: 8.4 for Nurses, 7 for Social Workers
