Basic Support Services: Writing the Support Plan Addendum 💻

It is recognized that 245D licensed basic support services do require less documentation in certain areas compared to intensive support services. However, basic support service providers are still required to have Support Plan addendum information for each person supported. This information directs the services and supports that each person receives from the provider and must be person-centered and include positive support strategies.

This session is designed specifically for 245D basic support service providers. We will explore the 4 Ws (who, what, when, and why) and the how to complete Support Plan addendum specific paperwork.

Course objectives:

• Learn what the Support Plan and Support Plan addendum are and who develops them
• Discuss timelines for when the addendum is developed, reviewed, and updated
• Understand what is required to be included in the addendum
• Discover writing techniques to document required information
• Review examples of documented Support Plan addendum information
