The Self-Management Assessment (SMA) is an important, but often misunderstood and underutilized, document. Required for all 245D Intensive Service recipients, the SMA is designed to provide a clear, person-centered picture of each person’s strengths, skills, and abilities, creating a strong foundation for effective, high-quality supports. This course provides practical instruction on how to write an accurate, compliant SMA that not only meets requirements, but also strengthens the services you provide through person-centered practices. You’ll explore each required area of the SMA and learn how to assess strengths, skills and abilities, and behaviors that impact a person’s ability to self-manage. The course also highlights common documentation errors and offers guidance on how to develop meaningful supports based on the information captured in the document.
Course Objectives:
This course is designed for Designated Coordinators/Designated Managers or other positions who are responsible for developing addendum documents for people supported.
CEU Contact Hours: 1.8 for Nurses, 1.5 for Social Workers
Registered participants will have access to the session recording for two weeks following the training.
