Imagine being introduced to new co-workers and the only things shared about you were negative! Your past bad decisions, mistakes you made, rumors about you, psychological profiles, and any negative personality trait. Then imagine these were all turned into a label that people only knew you by! Based upon information shared, your new co-workers would form a very negative, even hostile, picture of you. But this isn't who you are! So, how would this make you feel? Alone? Angry? Depressed? Isolated? Unfortunately, this is exactly what the people we support experience. People are often labelled by their conditions, behaviors, and past without many people taking the time to truly get to know them beyond those labels. One Page Description is a positive, eye-opening introduction to a person. It always includes three major topics: what is important to a person, what people like and admire about them, and how the person wants to be supported. This document can contain so much more and be used for a variety of purposes that goes well beyond the minimum of getting to know the person.

Course objectives:

  • Identify who a One Page Description is for (hint: everyone)
  • Learn the intention of the three major areas
  • Discuss how to gather information to develop these key areas and document them to build a One Page Description
  • Understand Important To vs Important For and how to find the balance between
  • Learn strategies to document what people like and admire about the person
  • Recognize how to listen to what support the person wants and not what others think the person needs
  • Learn additional person-centered tools that can be used in when gathering positive information with and about the person

During this course, you will learn how to truly listen to a person when gathering information to include in each section. One Page Descriptions are fun and an amazing to deepen your knowledge of a person-it is not and never should be "just one more form to do." Come and discover how you can truly change someone's life through the use of this simple, but effective person-centered tool.

CEU Contact Hours: 2.4 for Nurses, 2 for Social Workers
